By accessing or using services offered by Optimized Movement Rehab, PLLC (“Optimized Movement Rehab,” “we,” “our,” or “us”), you agree to be bound by these Terms and Conditions (“Terms”), including any documents incorporated by reference. If you do not agree with these Terms, please do not use our services.

Optimized Movement Rehab provides physical therapy and human performance services, including in-person evaluations, manual therapy, corrective exercise, and wellness education. Services may be delivered on-site or at affiliated partner locations.

You must be at least 18 years old or have the permission of a parent or legal guardian to participate in services. By receiving care, you affirm that you are physically able to participate or have received medical clearance to do so.

Appointments may be scheduled online or directly with our clinic. We request a minimum of 24 hours' notice for cancellations. Late cancellations or no-shows will be subject to a cancellation fee as outlined in your intake agreement or payment policy.

Payment is due at the time of service unless otherwise arranged. We accept cash, credit/debit cards, and Health Savings/Flex Spending Accounts. Packages and memberships are non-refundable and expire as stated at the time of purchase.

Optimized Movement Rehab provides physical therapy and wellness services but does not provide medical diagnoses beyond the physical therapy scope of practice. Any health information shared is intended to support — not replace — medical advice from your physician. If you experience pain or discomfort during any session, notify your provider immediately.

By participating in our services, you acknowledge the inherent risks of physical activity and voluntarily assume all responsibility for any injury, illness (including exposure to communicable diseases), or damages that may result. You release Optimized Movement Rehab, its staff, and affiliates from all liability arising from your participation, except where prohibited by law.

We are committed to protecting your health information as outlined in our HIPAA-compliant Notice of Privacy Practices, which is available at our office and on our website. By receiving care, you consent to the use and sharing of your information for treatment, payment, and healthcare operations.

We expect all patients and staff to maintain a respectful and professional environment. Harassment, discrimination, or inappropriate conduct may result in service termination.

All educational materials, videos, exercise programs, and branded content provided by Optimized Movement Rehab are protected by copyright and may not be reproduced or distributed without written permission.

You agree to indemnify and hold harmless Optimized Movement Rehab, its employees, and affiliates from any claims or demands, including legal fees, arising out of your use of our services or your breach of these Terms.

To the fullest extent permitted by law, Optimized Movement Rehab will not be liable for indirect, incidental, or consequential damages arising from the use of our services. Our total liability will not exceed the amount paid for services within the twelve (12) months preceding a claim.

These Terms shall be governed by the laws of the State of Texas. Any disputes shall be resolved through confidential binding arbitration in Travis County, Texas, in accordance with the rules of the American Arbitration Association. Each party waives the right to a jury trial and to participate in class actions.

We may revise these Terms from time to time. Updates will be posted on our website or made available at our office. Continued use of our services after such updates constitutes acceptance of the new Terms.

By purchasing a Physical Therapy (PT) Visit Package from Optimized Movement Rehab, you agree to the following terms and conditions:

PT Visit Packages are available only to active clients of Optimized Movement Rehab.

• New clients who have not completed an initial evaluation will be charged an additional $50 fee at the time of package purchase to cover the cost of the initial assessment.
  
  
• “Active client” is defined as any client who has completed an initial evaluation within the past six months and remains under an active plan of care.
  
  

• All package visits must be used within one (1) calendar year from the date of purchase.
  
  
• Unused visits after the one-year expiration date will be forfeited and are non-refundable.
  
  
• Packages are non-transferable and may only be used by the individual client who purchased the package.
  
  

• Clients are responsible for scheduling their own appointments through the online scheduling portal.
  
  
• Appointment availability is not guaranteed and is based on therapist scheduling capacity. Early scheduling is recommended to ensure preferred times.
  
  

• Clients must provide at least 24 hours’ notice to cancel or reschedule an appointment.
  
  
• Late cancellation or no-show will result in the loss of one (1) package visit and a $100 fee charged to the card on file.
  
  
• Repeated offenses may result in suspension of package privileges or scheduling access.
  
  

• PT Visit Packages are non-refundable and non-transferable under all circumstances, including changes in personal schedule, relocation, or unused sessions.
  
  
• Exceptions may be considered at the discretion of Optimized Movement Rehab for verified medical or emergency situations preventing participation.
  
  

• Korrect Fitness members receive discounted rates on physical therapy services, including packages.
  
  
• Membership must be active at the time of package purchase to receive discounted pricing. Retroactive adjustments will not be made.
  
  

• Package sessions must be used for services within the therapist’s scope of practice and may not be applied toward wellness, group sessions, or non-PT services unless otherwise stated.
  
  
• Consistent attendance and adherence to the plan of care are required to maintain clinical continuity and achieve optimal outcomes.
  
  

Optimized Movement Rehab reserves the right to modify these terms and conditions at any time. The most current version will always be available at optimizedmovementrehab.com and will apply to all future purchases.
